Medical Residency Letters of Intent: What to Include

A residency letter of intent should communicate a genuine preference clearly and professionally. It should follow the program’s communication rules and the current Match policies.

Check Whether Communication Is Welcome

Read the program’s and specialty’s instructions before sending a letter. Some discourage post-interview communications. Respect those limits rather than assuming another message will help.

Be Precise About Your Intent

A statement that you will rank a program first should be true and reserved for your actual first choice. If you are expressing interest without that commitment, say so accurately. Do not ask for a reciprocal ranking promise.

Explain the Specific Fit

Refer to features you learned about through research or the interview and connect them to your training goals. Include a relevant, accurate update if you have one. Keep the message focused rather than repeating your entire application.

Use the Current Match Calendar

Ranking and certification dates change by cycle. Follow the NRMP’s current ranking guidance and calendar instead of an old January-to-February schedule. Certify your list by the applicable deadline.

Rank by Your True Preferences

Your communication is separate from the rank order list. Follow NRMP instructions and rank programs in the order you actually prefer them. A favorable message from a program does not guarantee a Match result.
